A potential cause for this issue may be a Data corruption issue associated with the state of the workflow instance.

Restart the Forefront Identity Manager Service to retry the workflow recovery.  If the problem continues, cancel the Request associated with the problematic workflow instance.  If the Request cannot be cancelled, contact product support.
