Creates all databases described in the configuration file (primary import, 
star schema, archiving, analysis, and alerts) if they do not already exist. 
After the databases are created, all BAM metadata tables and stored procedures
are created.

{3} and {4} are required only if BAM alerts are being set up.
Only {3} can be specified on the command line, in which case {0}  
outputs a password prompt and reads the password from the console.
